What is CCYP?

CCYP bridges the gap between foster care and adulthood, providing practical support, life skills training, and consistent relationships during one of the most important transitions in a young person's life.

Young people aging out of foster care often step into adulthood without the basics most young adults take for granted: documents, transportation, job skills, stable housing or a trusted adult to call when something goes wrong.

Who qualifies?

CCYP may be available to youth ages 17–26 who are currently in foster care or have aged out of foster care and live in one of CMFCAA's 24 counties.

How does CCYP help?

CCYP provides voluntary case management that starts with immediate needs, then builds real-world skills and a support system.

  1. Secure immediate needs

We help youth secure immediate needs such as housing, identification documents, food access, transportation, and referrals for a variety of services.

  2. Build real-world skills

We work alongside youth to build skills like budgeting, cooking, job applications, mock interviews, college enrollment, and navigating public assistance.

  3. Build a support system

Our team helps youth identify trusted adults, reconnect with safe community members, and experience what it feels like to be supported without conditions.

What areas does CCYP cover?

CCYP can help with transportation, mental health, employment, education, financial, physical, and legal needs. These are skills many young adults learn while living at home, but youth aging out of foster care may not have had the same support or stability.

Social Support

We help youth identify trusted adults, reconnect with safe family members or friends, and experience what it feels like to be supported without conditions. No young adult should have to navigate adulthood alone.

Housing

We help youth find safe, affordable housing. This may include help with budgeting, apartment hunting or navigating housing referrals.

Education

Some youth know exactly what they want to do and need help making it happen. Others are still figuring it out. We advise on career assessment, ACT prep, financial aid, college enrollment, and college visits.

Physical and Mental Health

We help youth find a primary care physician, understand how Medicaid works, and access therapy, medical care, or prescription support.

Employment

Landing a job takes more than wanting one. We help youth build resumes, complete applications, prepare for interviews, and connect with employment services.

And More

Becoming an adult can be overwhelming. CCYP helps young people build independence one step at a time, from getting a driver's license to learning how to manage everyday responsibilities.

Does CCYP participate in HMIS?

Yes. CCYP participates in the Homeless Missourians Information System (HMIS).
